I agree that you have to write that stuff carefully. The reason that it works is because it is Ianto, because he is in control, and Jack is careful. So yes it could easily read like "you old perv" If it were Jack with a random 16 year old it would be even more difficult. In real life I wouldn't trust a man in his 40's with a 16 year old boy having a sexual relationship. Of course they can do it, but I think any of us would wonder what the Hell was going on. And yes I would feel the same if the younger one was a girl.
I think you are being very thoughtful to your readers, and yes it makes sense to warn if you don't want people reading and being offended. I do think it's not underage if the characters are in a country where it isn't. It doesn't matter where the reader is from.
And yay, I knew he was 70. I think that's an important plot point.
